Czech ordinary passport holders normally need a visa and may apply for an Azerbaijan e-Visa online. The Czech Ministry of Foreign Affairs describes the electronic route as single-entry and distinguishes it from an embassy visa process. Czechia, Czech Republic and the number the form actually needs
The nationality selector on this site uses Czechia. An older page or itinerary may use Czech Republic, but that naming difference does not mean a different passport category. Select the nationality of the passport being presented, not the country where you currently live or the airport from which you depart.
For the travel-document number, read the current passport itself. A rodné číslo is not the passport number, and an identity-card record is not a substitute for the passport page. Keep the readable image open while completing the form so that the number, name and issue and expiry dates can all be checked against the same source.
Map Czech name and birthplace fields without improvising
Enter surname and given names from the passport’s labelled identity fields rather than copying an unlabelled full name from a customer account. If accents, spacing or several given names create uncertainty, resolve how the field should be entered before submitting. Do not create a new identity spelling simply to make an unrelated website’s saved profile agree.
Treat birthplace and current residence as separate questions. Moving abroad does not change the birthplace printed in the passport, and holding a Czech residence card does not make a foreign passport Czech. In a mixed-nationality household, check each traveller’s actual document instead of applying this guide to the whole booking.

Decide whether a single-entry e-Visa fits the itinerary
Czech foreign-ministry guidance describes two distinct routes: a single-entry electronic visa and a consular application through the Azerbaijani representation. Do not combine their document lists or assume a fee quoted for one process describes the other. These evis.az packages are for the electronic single-entry application service.
A short Baku visit without leaving and returning may fit that entry pattern. A Baku–Georgia–Baku journey requires a separate review of the return entry; the unused days on a single-entry visa are not a reusable balance. Clarify a multiple-entry or other-category requirement before buying a short-visit package.
Compare passport expiry with the end of the visa window
Czech travel-document advice requires a passport and discusses validity beyond planned departure. The e-Visa condition uses at least three months after visa expiry, which is a different reference date. For the electronic application, compare against the visa end date rather than relying only on the day a one-week holiday finishes.
Complete a planned passport renewal before entering the replacement number. Check that a saved image and the document you intend to carry match the form. If a passport is lost, replaced or otherwise no longer usable after the visa is issued, do not assume the old permission can simply be used with a different number.
Choose timing, review the result and keep a printed copy
Standard is $59 USD per applicant, with a three-working-day processing window. Urgent is a separate $99 total, with a three-hour option including weekends and holidays. These are not CZK quotations. Use the actual local arrival in Azerbaijan from your flight itinerary and allow preparation time before a complete submission.
The result is delivered electronically, but an application reference is not an issued visa. Once the PDF arrives, check the name, passport number, entry dates and permitted stay, then print it. Prepare a separate record for every child. For a stay exceeding 15 days, confirm who will complete place-of-stay registration rather than assuming it comes with the visa.

The evis.az application lists Czechia. Use that option for the Czech passport route; the Czech Republic wording found in older sources does not represent a different nationality option here.
Is my rodné číslo the number needed for an Azerbaijan e-Visa?
No. In the passport-number field, use the document number from the current passport being uploaded and carried. Do not substitute a personal identifier or identity-card number.
Can I obtain multiple entries through this Czech passport e-Visa page?
The normal electronic visa is single-entry. Czech consular guidance distinguishes it from an embassy process that can cover other visa needs. Resolve repeated-entry plans before choosing a package here.
