The short answer
Can Sri Lankan citizens apply for an Azerbaijan e-Visa?
Yes. Sri Lanka is on Azerbaijan’s e-Visa eligibility list for an eligible visit using an ordinary passport. Sri Lankan passport holders normally need a visa; the usual e-Visa is single-entry and permits up to 30 days within a 90-day validity window. Your passport and issued visa must agree on the traveller’s identity and document number.

The electronic route and your Sri Lankan passport
The official eligibility list includes Sri Lanka. The Azerbaijani Embassy in New Delhi describes the electronic visa as a separate process and explains that the embassy does not issue or amend e-Visas. Do not interpret a general consular appointment checklist as proof that every eligible electronic applicant must attend an embassy.
Sri Lanka’s Immigration Department describes current ordinary passports as valid for all countries unless otherwise specified. Inspect your own document, including any endorsement, instead of assuming an emergency certificate or restricted document has the same use. If replacing an older passport, wait until the document intended for this trip has been issued before entering its number.
Use the passport’s full names, not an initial-based shortcut
Keep the identity page open while completing surname and given-name fields. A Sri Lankan name used with initials at work or in an email address may look different from the full entries in the passport. Read the labelled fields and use the Latin spelling in the machine-readable lines as the form requests, rather than deciding the identity from an informal signature.
The NIC and passport number are different identifiers. For the travel-document field, read the passport number from the same page you upload. If the passport has no surname, this form allows the surname field to remain blank; do not invent FNU, XXX or a dash. Ask before submitting if a long name cannot be represented accurately rather than discarding part of it.

Prepare the actual upload rather than a bundle of unrelated papers
For the evis.az form, prepare a readable image of the passport identity page and the requested travel, personal, contact and accommodation details. Use a JPG or PNG accepted by the upload field. Photograph the complete page without glare or fingers covering the document number and date fields; check readability before continuing.
A NIC scan, bank statement or photo of the passport cover does not replace that identity page. A separate portrait is not the passport-page upload. Information you keep for transit, airline checks or another visa process should not be confused with an extra mandatory attachment in this form. Use your real accommodation address in Azerbaijan and an email you can monitor.
Plan processing from the final arrival, not only the Colombo departure
Read the final flight into Azerbaijan after any connection or overnight stop. Use that local arrival when choosing the visa start date. If an itinerary changes, compare the new arrival with the permission actually issued; a receipt for an application is not evidence that entry is authorised on the revised date.
Standard costs $59 USD per applicant, with a three-working-day processing window. Urgent is $99 USD in total, with a three-hour option including weekends and holidays. These are USD service package totals, not LKR quotations. Complete document preparation first and allow time to receive, inspect and print the issued visa before departure.
Check expiry and each traveller’s result before flying
The passport must remain valid for at least three months after the e-Visa expires. Count from the visa end date, which can be much later than the last night of a short holiday. Do not mix the issue date of a renewed passport with the expiry or image of the old one.
Review each adult’s and child’s own passport record before payment. After issuance, print the PDF matching that person and check entry dates and permitted stay. The 90-day window is not a 90-day stay or repeated-entry permission. If the visit exceeds 15 days, arrange place-of-stay registration separately with the hotel or host.

Questions from Sri Lankan travellers
Is a Sri Lankan passport eligible for an Azerbaijan e-Visa?
Sri Lanka appears on the official eligibility list. Use your actual ordinary passport and check your travel purpose and any individual exception before choosing the electronic route.
Do I have to visit an embassy for an eligible Azerbaijan e-Visa application?
The electronic route is separate from an embassy visa application. The Azerbaijani Embassy in New Delhi says it does not issue or amend e-Visas. A non-standard document or different visa category still needs an individual check.
What if my Sri Lankan passport has no surname or I normally use initials?
Use the passport’s name fields and the Latin spelling in its machine-readable lines. This form allows a blank surname when the passport has none; do not add FNU, XXX or a dash. Do not shorten full given names to the initials used informally.
Do I upload my NIC or a separate portrait?
The passport upload in this form requires the passport identity page. Neither a NIC image nor a separate portrait replaces it. Prepare a readable JPG or PNG and follow the upload field’s displayed requirements.
Does living in the UAE let me select UAE instead of Sri Lanka?
Select the nationality of the passport used for travel. Residence in the UAE does not turn a Sri Lankan passport into a UAE passport. Check any qualifying residence-based entry arrangement separately rather than changing your nationality in the form.
